Skagit County Table of Ten

Core Strategies
1. Provide immediate and frequent visitation using a coaching model.
2. Achieve full collaboration and participation by all.
3. Upon removal of each child, proceed with urgency.
Critical Behaviors
1. Provide meaningful opportunities for parents, family, attorneys, GAL / CASA, service providers, CHET screeners, social workers and their supervisors, placements, and youth when appropriate, to be involved in 10 day case conferences.
2. At each ten day case conference, meaningfully address in concrete terms placement, visitation, services for parents and kids, transition issues, and what the next steps are in the process.
3. Develop WISH family journal technology.
Measure of Success
The county will measure success by examining the reduction of time in care with no completed plan. Base-line data points as of 9-25-08 are as follows:
